- Applicants must select the appropriate District/Constituency (GP/VCDC/WARD, etc.). If any GP/VCDC, etc., is not available in the list, please contact the respective District Sports Officer.
- Please provide a valid mobile number. Without a valid mobile number, registration may be subject to cancellation.
- Applicants must fill out the form and upload the required supporting documents.
- After the successful submission of the form, an acknowledgment will be generated containing the application reference number (Unique ID). For further communication, use this Unique ID and mobile number.
